You can complete the Questionnaire with the free Adobe Acrobat Reader, available at www.adobe.com. Once you open the form, do a “Save As” with your last name as the form name. When you complete the form, click the “Submit” button on page 3 and select the appropriate method to send the form. Note: You may have to print it out and mail or deliver it to our office, depending on the email program you use.
